• The health history form is required to be filled out and sent in at least 30 minutes before our session. Notebook and pen are optional but recommended.

  • We will discuss your health history form which talks about past and current health. We will also talk about what will happen moving forward and if we are a good fit!

  • Please set aside 15-30 minutes to fill this form out. This health history serves as a starting point to the coaching program, and will be used to track progress. The form allows for pattern and behavior repetition to be recognized so change can take place.
